Documentation coverage
A capability counts as documented when at least one real page is written about it. A stub does not count: it marks a gap, so the number moves only when the gap is actually filled.
For each app, file coverage is the share of that app's source files a page says it documents through its `covers:` list. A page that covers a directory documents every file beneath it, so a directory counts for all of them, not as one.
